An HMPA catalyzed reductive alkylation of ureas and thioureas with trichlorosilane under mild reaction conditions has been developed. Both aldehydes and ketones could be used as efficient alkylation reagents in this method to obtain the desired products in moderate to high yields. A variety of di- and trisubsituted ureas and thioureas were easily prepared by this new reductive alkylation method. This protocol exhibits excellent functional group tolerance, chemoselectivity and practicality.

Objective: To explore the relationship between meteorological factors and scarlet fever incidence from 2006 to 2017 in Guangzhou, the largest subtropical city of Southern China, and assist public health prevention and control measures. Methods: Data for weekly scarlet fever incidence and meteorological variables from 2006 to 2017 in Guangzhou were collected from the National Notifiable Disease Report System (NNDRS) and the Guangzhou Meteorological Bureau (GZMB). Distributed lag nonlinear models (DLNMs) were conducted to estimate the effect of meteorological factors on weekly scarlet fever incidence in Guangzhou. Results: We observed nonlinear effects of temperature, relative humidity, and wind velocity. The risk was the highest when the weekly mean temperature was 31 degrees C during lag week 14, yielding a relative risk (RR) of 1.48 (95% CI: 1.01-2.17). When relative humidity was 43.5% during lag week 0, the RR was 1.49 (95% CI: 1.04-2.12); the highest RR (1.55, 95% CI: 1.20-1.99) was reached when relative humidity was 93.5% during lag week 20. When wind velocity was 4.4 m/s during lag week 13, the RR was highest at 3.41 (95% CI: 1.57-7.44). Positive correlations were observed among weekly temperature ranges and atmospheric pressure with scarlet fever incidence, while a negative correlation was detected with aggregate rainfall. The cumulative extreme effect of meteorological variables on scarlet fever incidence was statistically significant, except for the high effect of wind velocity. Conclusion: Weekly mean temperature, relative humidity, and wind velocity had double-trough effects on scarlet fever incidence; high weekly temperature range, high atmospheric pressure, and low aggregate rain fall were risk factors for scarlet fever morbidity. Our findings provided preliminary, but fundamental, information that may be useful for a better understanding of epidemic trends of scarlet fever and for developing an early warning system. Background Accurate reflux episode identification is crucial for pH-impedance interpretation. Individual reflux episode characteristics associated with inter-reviewer concordance are incompletely understood. Methods Ambulatory pH-impedance studies from 19 GERD patients (median age 52 years, 78.9% F) were analyzed by 5 reviewers. Metadata from pH-impedance studies were exported to a dedicated software tool designed to compare episode-by-episode identification between reviewers within a +/- 7.5 s window. Patient position, acidic vs. nonacidic episodes, acid clearance time (ACT), bolus clearance time (BCT), and proximal extent of reflux episodes were compared between episodes identified by all reviewers against those identified by automated analysis, and one to four reviewers, respectively. Results Automated analysis identified 1644 episodes (median 78 episodes per patient, IQR 64-108), of which 84.9% were identified by >= 3 reviewers and 57.1% by all reviewers; 339 unique episodes were added by at least 1 reviewer. Characteristics defining 5 reviewer concordance included acid reflux episodes (88.9%), upright episodes (88.4%), high proximal extent (median 17 cm, IQR 15-17 cm), and longer acid clearance times (67.0 s, IQR 29.0-146.0 s) (P < 0.001 compared to 1-4 reviewer concordance for each). In contrast, 1 reviewer-identified episodes were 69.8% acidic, 76.9% upright, and limited to the distal esophagus. Using 5-reviewer concordance, designation of GERD evidence changed from automated analysis in 16%-19% of patients. Conclusions Acidic episodes with high proximal extent in the upright position and longer acid clearance times on pH-impedance studies have the highest concordance for identification by expert reviewers. The generation of a methyl carbon source from renewable and cheap sources is challenging. Herein, we describe a novel and an efficient route for methylation and acetylation of aza-heteroarenes using PEG-400 under O-2 and TsOH center dot H2O for the first time by tuning the reaction conditions using a different set of starting materials. The key features of the current protocol are oxidative C-O and C-C bond scission under metal-free conditions with good functional group tolerance, and a broad substrate scope. The potential applicability of the designed methodology was demonstrated for the synthesis of central nervous system (CNS) depressant and anticonvulsant drug molecules by a one-pot strategy.

BACKGROUND AND OBJECTIVES: Latino children in immigrant families experience health care disparities. Text messaging interventions for this population may address disparities. The objective of this study was to evaluate the impact of a Spanish-language text messaging intervention on infant emergency department use and well care and vaccine adherence. METHODS: The Salud al Dia intervention, an educational video and interactive text messages throughout the child’s first year of life, was evaluated via randomized controlled trial conducted in an urban, academic pediatric primary care practice from February 2016 to December 2017. Inclusion criteria were publicly insured singleton infant < 2 months of age; parent age > 18, with Spanish as the preferred health care language; and at least 1 household cellular phone. Primary outcomes were abstracted from the electronic medical record at age 15 months. Intention-to-treat analyses were used. RESULTS: A total of 157 parent-child dyads were randomly assigned to Salud al Dia (n = 79) or control groups (n = 78). Among all participants, mean parent age was 29.3 years (SD: 6.2 years), mean years in the United States was 7.3 (SD: 5.3 years), and 87% of parents had limited or marginal health literacy. The incidence rate ratio for emergency department use for the control versus intervention group was 1.48 (95% confidence interval: 1.04-2.12). A greater proportion of intervention infants received 2 flu vaccine doses compared with controls (81% vs 67%; P = .04). CONCLUSIONS: This Spanish-language text messaging intervention reduced emergency department use and increased flu vaccine receipt among a population at high risk for health care disparities. Tailored text message interventions are a promising method for addressing disparities.

A water-applicable hexamethylbenzene (HMB) chemosensor, (E)-2-((2-hydroxy-3-methoxybenzylidene)amino)benzamide, displayed a selective colour change from colourless to yellow in the existence of copper ions. The detection limit was computed as 1.57 mu mol/L, which is substantially lower than the World Health Organisation guideline (31.5 mu mol/L) for copper ions. Probe HMB could also sense and quantify copper ions in real water samples. The coordination mode of HMB to copper ions was demonstrated through Job plot, electrospray ionisation mass spectroscopy and proton nuclear magnetic resonance titration, and was also supported by theoretical studies.

Spain is the first olive oil producer worldwide (representing around 45%), with more than 80% of its agricultural area dedicated to this crop. The goal of this study is to assess the environmental impact of the Spanish virgin olive oil production, attending to the farming and industrial phases. A Life Cycle Assessment (LCA) is conducted for 5 harvests in approximately 4.000 ha of olive grove in Jaen, the largest producer region of Spain. The type of farming is the most representative in Spain: conventional (non-organic), with a medium-low slope, extensive (100-150 trees per ha) and around 60% dryland orchards. The industrial phase is based on 2-phase extraction process and olive pomace valorization. The functional unit (FU) chosen for the comparative analysis is 1 kg of unpacked virgin olive oil under a perspective from cradle to gate. One of the most representative categories, climate change, places the average environmental impact in 2.43 kg CO2 eq/kg, while the range is between 1.93 and 3.00 kg CO2 eq/kg depending on the harvest. Huge differences between values are observed in the farming phase and they are mostly caused by the virgin olive oil yield of every harvest. The carbon sequestration analyses and the impact produced per FU ranges from 43.78% (for 15/16 harvest) to 46.36% (for 17/18). The impact hotspots detected in the farming phase, in terms of climate change potential, are the categories of plant protection products and herbicides (24.11% of the average value) and fertilizers (20.01%). However, the environmental impact of the industrial phase is relatively constant, with a value of 0.547-0.554 kg of CO2 equivalent. Soft ionic conductors, such as hydrogels and ionogels, have enabled stretchable and transparent ionotronics, but they suffer from key limitations inherent to the liquid components, which may leak and evaporate. Here, novel liquid-free ionic conductive elastomers (ICE) that are copolymer networks hosting lithium cations and associated anions via lithium bonds and hydrogen bonds are demonstrated, such that they are intrinsically immune from leakage and evaporation. The ICEs show extraordinary mechanical versatility including excellent stretchability, high strength and toughness, self-healing, quick self-recovery, and 3D-printability. More intriguingly, the ICEs can defeat the conflict of strength versus toughness-a compromise well recognized in mechanics and material science-and simultaneously overcome the conflict between ionic conductivity and mechanical properties, which is common for ionogels. Several liquid-free ionotronics based on the ICE are further developed, including resistive force sensors, multifunctional ionic skins, and triboelectric nanogenerators (TENGs), which are not subject to limitations of previous gel-based devices, such as leakage, evaporation, and weak hydrogel-elastomer interfaces. Also, the 3D printability of the ICEs is demonstrated by printing a series of structures with fine features. The findings offer promise for a variety of ionotronics requiring environmental stability and durability.
